A place where people feel known, safe and at home
Support that fits around real lives
Every person is different. That’s why we build support around the individual, not the other way around. You’ll see familiar faces, clear routines and a team that takes the time to understand what matters, so people feel safe, confident and able to grow in their own way.
Support with daily routines, work and living independently in a place that feels like home.
Flexible, community-based support that adapts to changing needs and helps people stay connected.
Purpose built homes with on-site support close by, so people can live independently without having to manage alone.
Reliable, personalised care delivered in familiar surroundings by people who know you.
Helping people develop skills, build confidence and find meaningful work at their own pace.
Structured daytime activities that build skills, grow confidence and keep people connected to the people and places that matter.

Testimonials
At Mediline, you really see the impact of what you do. Whether it’s supporting someone to move into more independent living or helping them build everyday skills, it’s about giving people the chance to live their lives in the way they choose.
I started as a support worker and I’ve grown through every level since. Mediline invested in me at each step, qualifications, training, development, and that’s not something you take for granted. The culture here is honest and it’s a team effort from the support workers right through to leadership. Everyone pulls in the same direction, and you feel that.

How Referrals Work
Most referrals come through local authority teams, social workers, brokerage teams and commissioners. We also welcome enquiries from case managers and families managing direct payments.
If you are a social worker or commissioner, referrals usually come through your local authority process. If you are contacting us directly, we will guide you through the next steps and make sure the right people are involved from the start.
Step 1: Get in touch
Reach out directly or through your local authority referral process. We are on framework provider lists across our regions and receive referrals via secure platforms including Zivver and The Chest.
Step 2: We review the referral
Our team reviews the information you provide. If we need anything further, we’ll come back to you quickly to make sure we have a clear picture.
Step 3: We come to you
If supported living looks like a good fit, we arrange a detailed assessment with the individual and the people involved in their support.
Step 4: The person decides
The pre-tenancy assessment is led by an experienced clinician. We take time to understand the person’s needs, strengths and what matters most to them.
Step 5: Transition and move in
We share a clear assessment report and, where appropriate, agree next steps and a carefully planned transition into support. Nothing is rushed.
